These Ad-Hoc sessions on 11 Aug or 18 Aug are for students who are unable to attend IDs Verification & Card Collection from 25 July 2017 - 27 July 2017.

Note:

  • Only students who have matriculated and submitted their photograph online are eligible to attend the Ad-Hoc IDs Verification & Card Collection Day!
  • For students who are attending the Ad-Hoc IDs Verification on 11 Aug, please complete your medical check-up by 4 Aug 2017.
  • For students who are attending the Ad-Hoc IDs Verification on 18 Aug, please complete your medical check-up by 14 Aug 2017.

  • Singaporeans  : NRIC or Passport 
  • Singapore PRs: Blue NRIC, Passport AND Re-entry Permit 
  • International Students: Passport AND Dependent/Student Pass
  • Pre-enrolment Medical Report (RMG)
  • ^SMU Indemnity Form

You will not be allowed to collect your SMU Student Card if the COMPLETE set of original documents is not produced.

  • Please bring both original and photocopy of all the identification documents.

^SMU Indemnity Form: applicable to students below 18 years of age at the point of online matriculation. The Indemnity form is available for printing when you complete your online matriculation.

All students are to update their Singapore bank details online by logging in to OASIS > Home > Policies and Self-Service Apps > Maintain Bank Accounts > Credit Bank Account for any reimbursement during the course of study.

Students who do not have any Singapore-based bank account* are required to open the account and update the bank details online by end August.

*This refers to bank accounts maintained with the Singapore branch.

3.

Tuition fee-related forms (if any)

All students are encouraged to pay their fees by Interbank GIRO. Please update your bank account details online by logging in to OASIS > Home > Policies and Self-Service Apps > Maintain Bank Accounts > Debit Bank Account. 

Please note that the signed copy of the Interbank GIRO form and all the forms listed below should be submitted to Office of Finance via “Finance Office” drop box by end August 2017. Please also note that GIRO payment will only commence from AY2017-18 Term 2 for successful applicants.

CPF Declaration Form (after your online submission direct to CPF Board)
