Part 2: Checklist for Verification & Card Collection Day (International Student)

Kindly come down to SMU in person with the following COMPULSORY documents:

Note:

  • Only students who have matriculated and submitted their photograph online are eligible to attend the IDs Verification & Card Collection Day!
  • Please complete your medical check-up by 19 July 2017
  Compulsory Documents Remarks

1.

International Students

  • Passport
  • Student’s Pass (ICA)
  • Pre-enrolment Medical Report (RMG)
  • ^SMU Indemnity Form

You will not be allowed to collect your SMU Student Card if the COMPLETE set of original documents is not produced.

Please bring both original and photocopy of all the identification documents.

^SMU Indemnity Form: applicable to students below 18 years of age at the point of online matriculation. The Indemnity form is available for printing when you complete your online matriculation.

All students are to update their Singapore bank details online by logging in to OASIS > Home > Policies and Self-Service Apps > Maintain Bank Accounts > Credit Bank Account for any reimbursement during the course of study. (Please refer to Step 7 "Update Bank Information" at International Student Matriculation home page.)

Students who do not have any Singapore-based bank account* are required to open the account and update the bank details online by end August.

*This refers to bank accounts maintained with the Singapore branch.

3.

Tuition fee-related forms (if any)

All students are encouraged to pay their fees by Interbank GIRO. Please update your bank account details online by logging in to OASIS > Home > Policies and Self-Service Apps > Maintain Bank Accounts  > Debit Bank Account. (Please refer to Step 7 "Update Bank Information" at International Student Matriculation home page.)

Please note that the signed copy of the Interbank GIRO form and the listed form below should be submitted to Office of Finance during the IDs Verification & Card Collection Day, or via “Finance Office” drop box by end August 2017. Please also note that GIRO payment will only commence from AY2017-18 Term 2 for successful applicants.
