PART 2: CHECKLIST FOR IDs VERIFICATION & CARD COLLECTION DAY (SUTD-SMU)

Kindly come down to SUTD campus in person with the following COMPULSORY documents:

·         Only students who have matriculated and submitted their photograph online are eligible to attend the IDs Verification & Card Collection Day!

For Singaporeans:

  • NRIC/Passport
  • ^SMU Indemnity Form

For Singapore Permanent Residents:

  • Blue NRIC
  • Passport
  • Re-entry Permit
  • ^SMU Indemnity Form

For International Students:

You will not be allowed to collect your SMU Student Card if the COMPLETE set of original documents is not produced.

Please bring both original and photocopy of all the identification documents.

^SMU Indemnity Form: applicable to students below 18 years of age at the point of online matriculation. The Indemnity form is available for printing when you complete your online matriculation.

The "Receiving Bank Account" is the account that you would like to receive payment from the University. This could be in the form of Bursaries, Scholarships, Stipends, Expense claims and other forms of official payment.
 

All students are to update their Singapore bank details online by logging in to OASIS Home > Policies and Self-Service Apps > Maintain Bank Accounts > Add Receiving Bank Account > Fill in the relevant details > Submit, for any reimbursement during the course of study. 

Students who do not have any Singapore-based bank account* are required to open the account and update the bank details online by end August.

*This refers to bank accounts maintained with the Singapore branch.

Fee-related forms (if any)

All students are encouraged to pay their annual fees by Interbank GIRO. 

Please apply online, print and submit the signed copy of the Interbank GIRO form during the “IDs Verification & Card Collection” day or via “Finance Office” drop box by end August 2017. Please also note that GIRO payment will only commence from AY2017-18 Term 2 for successful applicants. 

Steps to add “Paying Bank Account” details

Please ensure that you have the following information on hand before filling up your “Paying Bank Account” details

a.        Name of payee’s bank
b.        Payee’s bank account number
c.        Payee’s name as reflected in their bank statement or bank book

You will also require access to a printer to print out the generated form once it has been generated.

Login to Student Portal OASIS (https://oasis.smu.edu.sg)

Click on the Navigation path by order of: OASIS > Home > Policies and Self-Service Apps > Maintain Bank Accounts > Add Paying Bank Account > Fill in the relevant details > Submit > Print GIRO Application Form (this might take a few minutes to process) Print the generated PDF document > Payee to sign the document with signature as per bank records > Submit the signed form

You can either submit the form during the IDs Verification & Card Collection day or drop it into the Office of Finance drop box available at:

a. Level 1, SMU Administration Building (next to the Main entrance)
b. Level 12 , SMU Administration Building (next to the Finance Counter)
