Part 2: Checklist for Verification & Card Collection Day (Singapore Permanent Resident)

Kindly come down to SMU in person with the following COMPULSORY documents:

30 July: 2.00pm – 5.00pm
(1st day session starts at 2.00pm)
31 July: 9.00am – 11.30am / 2.00pm – 5.00pm
1 August: 9.00am – 11.30am / 2.00pm – 5.00pm

***For students who matriculated online from 21 – 24 July and – 4 Aug, please attend the following IDs Verification Session:

School of Accountancy/School of Law
Level 2, Seminar Room 2.4 (Room 2021)
15 August: 2.30pm – 4.30pm

Please take note that if you turn up for the IDs Verification from 30 Jul - 1 Aug, you will not receive your student card as it will not be ready for collection.

Note:

  • Only students who have matriculated and submitted their photograph online are eligible to attend the IDs Verification & Card Collection Day!
  • Please complete your medical check-up by 24 July 2014.
  • For students who matriculated online from 21 – 24 July and – 4 Aug, please complete your medical checkup by 6 Aug 2014.
 
  Compulsory Documents Remarks

1.

Singapore Permanent Residents

  • Blue NRIC
  • Passport
  • Re-entry Permit
  • Pre-enrolment Medical Report (RMG)
  • ^SMU Indemnity Form

You will not be allowed to collect your SMU Student Card if the COMPLETE set of original documents is not produced.

Please bring both original and photocopy of all the identification documents.

^SMU Indemnity Form: applicable to students below 18 years of age at the point of online matriculation. The Indemnity form is available for printing when you complete your online matriculation.

All students are to update their Singapore bank details online by logging in to OASIS > Personal & Finances > Maintain Bank Accounts > Credit Bank Account for any reimbursement during the course of study.

Students who do not have any Singapore-based bank account* are required to open the account and update the bank details online by end August.

*This refers to bank accounts maintained with the Singapore branch.

3.

Tuition fee-related forms (if any)All students are encouraged to pay their fees by Interbank GIRO. Please update your bank account details online by logging in to OASIS > Personal & Finances > Maintain Bank Accounts > Debit Bank Account.

Please note that the signed copy of the Interbank GIRO form should be submitted to Office of Finance (Station 5) during the IDs Verification & Card Collection Day, or via “Finance Office” drop box by end August 2014. Please also note that GIRO payment will only commence from AY2014-15 Term 2 for successful applicants.
